2018-2018 Faith Formation Registration

 You must be a registered member of St. Matthew Catholic Church to participate in
 Faith Formation. If you have not yet registered with the parish, please stop by the
 main church office in Ballantyne, Monday - Friday, between 8:30 and 4:30 for a
 welcome packet. These are also available at the Welcome Desk, before or after
 mass, at both Ballantyne and Waxhaw locations.

 *** If you don't know your envelope number, and are a registered member
 of St. Matthew parish, please type "0000" in the space provided
 (after selecting "Update Existing Family"). ***

 Online registration requires payment upon completion of the form, either by credit
 card, or ACH (bank draft). If you prefer to pay cash or need financial assistance,
 please stop by the main church office to register in person,
 Monday - Friday, 9:00 AM - 1:00 PM.
